As of late 2025, approximately one in five Americans (about 20-21%) are covered by Medicaid. This translates to over 70 million people, though enrollment can fluctuate. The program serves low-income children, families, adults, seniors, and people with disabilities.  

Overall population: About one in five Americans are covered by Medicaid.

Children: Medicaid covers a significant portion of children, with estimates suggesting around 40% of all children receive coverage.

Adults: Medicaid covers about one in six non-elderly adults. The percentage can be higher in states that have expanded Medicaid eligibility.

Seniors: Approximately 1 in 6 adults aged 65 or older are covered by Medicaid. A large portion of Medicaid spending goes to long-term care services for seniors and people with disabilities.

The most recent official enrollment data available from the Centers for Medicare & Medicaid Services (CMS) is for June 2025, which reported 70,506,786 people enrolled in Medicaid across the 50 states and the District of Columbia.

Enrollment levels declined in fiscal year (FY) 2025 due to the "unwinding" of the continuous enrollment provision that was in place during the COVID-19 pandemic.

Enrollment is projected to flatten or have very slight growth in FY 2026, but remains above pre-pandemic levels in many states.

There is significant variation in coverage rates across different states, largely due to differences in state-level policies like the Medicaid expansion under the Affordable Care Act (ACA).

Medicaid covers around 41% of all births in the U.S. and nearly 1 in 5 Medicare beneficiaries (dual-eligibles).
